101. The purpose of
this design standards technical manual (DSTM) is to ensure that all persons
performing work for the City of North Logan comply with the designated
standards, approved plans and specifications, and the City’s safeguards to
protect life and property.
102. Construction Checklist. The following checklist shall be used by all contractors performing work within the City to ensure that the standards within this manual are being complied with.
CONSTRUCTION CHECKLIST
All requests for inspection shall follow the procedure outlined in the DSTM manual sections 504-508. All testing is required to be provided by the contractor or its designated testing agent. Anyone needing a DSTM may purchase them from the city offices for a fee of $10.00. No work shall be performed on any project without an approved set of plans on the jobsite.
Work requiring “continuous inspection” requires three days advance notice and work requiring “periodic inspection” requires one day (24 hours) advance notice. Such notice shall be scheduled with the secretary at the city offices.
All development shall
be done in a unified and organized manner.
The order of improvements
are to
be done shall follow the order outlined herein, unless written permission to
change the order is granted by the appropriate city official. Such permission will only be granted under
special circumstances.
Plans:
q Contractor has a set of plans approved and signed by the City Engineer
q All plan changes have been approved by the city’s engineering department and redlined on the city’s construction drawings
Pre-Construction
Meeting:
q Meeting held with developer. Developer shall schedule this meeting with the city. The meeting shall include all contractors involved in the work, utility companies, and representatives from the city’s engineering, streets and utilities departments.
q All contractors have in their possession an approved set of construction drawings
Utilities:
q Shall include water, sewer, and storm drainage systems. These systems may be installed in any order or simultaneously, but must be complete prior to any initial base courses being installed for the roadway sections.
Water Lines:
q Part
Verification. The City inspector shall
verify that proper fittings, setters, piping materials, etc. are at the job
site prior to their installation.
q Trench
Backfill Compaction. A 100-foot test
area shall be tested at one-foot lift intervals the full depth of the
trench. This section shall be used to
establish the required compactive effort to reach proper compaction. The trench backfill shall then be
periodically tested at changes in soil type, or not to exceed a distance of 250
feet of trench length at one-foot lifts to verify the compactive effort.
q Verification

q Barrels
and setters installed properly. Setters
shall not exceed four feet below finished grade elevation. Five-foot, minimum length, barrels shall be
used in all cases.
Sewer Lines:
q Part

Storm Drainage Systems:
q Parts Verification. Ensure proper piping materials, drop boxes, catch basins, sedimentation and detention basins, etc.
q All lines have been cleaned with a sewer jet or other similar equipment.
q Hydrostatic or air tests of all pipe lines
q Video inspection of all pipe lines
q Inspection of all detention systems prior to backfill
q Detention systems sized per plans
Utility
Finalization:
q All utilities within street section are installed and approved
q All service laterals are installed and located on parcels with physical markers
q All underground and under-roadway drainage systems are installed and approved
q All backfill compaction tests have been submitted and approved
q All air tests have been submitted and approved
q Video inspections have been reviewed and approved
Street Sections:
q Street
sections shall be installed in the order shown herein.
q All base course compaction tests shall be taken at the locations specified herein as directed by the city’s engineering department
Prepare Sub-grade:
q Sub-grade scarified and re-compacted to a six-inch depth
q Sub-grade inspected and verified to be of acceptable material for roadway construction and compaction.
Initial Base
Courses:
q Granular barrow (pit run) installed at the required depth
q Compaction tests for granular borrow submitted. Ensure one test required for each lane width and under curb and gutter, to be taken at 250 feet intervals of project length, with a minimum of four tests on any project. Lane widths are left/center/right/etc. as defined by streets cross-section width.
Curb and Gutter:
q Compaction test results from authorized testing agency have been submitted and approved by the city engineering department for the initial base courses.
q Curb and gutter stakes set and checked for proper location and alignment
q Curb and gutter forms set and approved for location
q Drainage boxes verified and approved
q Drive approaches identified. Any approaches not installed at the time of installation of the curb and gutter shall be saw-cut when those approaches are to be installed.
q Testing
agency has contacted city engineer for testing procedures prior to first day’s
pour.
q Compressive
strength cylinder tests approved. One
test required for every 50 cubic yards or one test for each day’s pour if less
than 50 cubic yards.
q Slump tests approved. Required the same as cylinder tests. Results shall be as specified in DSTM, or the load shall be rejected.
q Air-entrainment test approved. Required the same as cylinder tests. Results shall be as specified in DSTM, or load shall be rejected.
q Expansion joints placed every 100 feet and ruled every ten feet
q Drainage boxes finished to proper grade with bicycle-safe grates
q Curb cuts approved. All curb cuts must be saw cut if not done initially with the curb.
Final Base
Courses:
q Slump and air-entrainment tests results from authorized testing agency submitted and approved. Cylinder tests shall be submitted upon break requirements.
q Crushed gravel (untreated base ¾” max. gradation) installed at required depth
q Compaction tests for crushed gravel submitted. One test required for each lane width, and Sidewalk, to be taken at 100-feet intervals of project length, with a minimum of eight tests on any project. Lane widths are left/center/right/etc. as defined by streets cross-section width.
q Proctor verification on materials submitted with all compaction tests
Drainage
Facilities:
q Drain inlet and outlet boxes installed at proper locations
q All drainage piping connects at curb boxes as designed
q Cross-drains are a minimum of five feet wide
q Inspector verifies cross-drains have a maximum 1.5” depth prior to laying concrete
q Inspector verifies cross-drains contain three each number five (#5) epoxy coated reinforcing steel bars placed three inches above the ground. Lap splices shall be a minimum of 15”.
Surface Course:
q Compaction test results from authorized testing agency have been submitted and approved by the city engineering department for the final base courses.
q Street grades and slope checked and approved prior to any asphalt being delivered to project
q Asphalt materials are ½” max. gradation
q Asphalt laid to required depth
q
q Manholes, valves, etc. raised to surface and concrete rings installed
Sidewalks:
q Sub-grade compacted. One test for each lot or every 500 feet of length if laid concurrent.
q Testing agency has contacted city engineer for testing procedures prior to first days pour.
q Sidewalk location verified
q Four inches of crushed rock verified
q Sidewalk slope verified once forms are set prior to concrete placement
q Expansion joints placed every twenty feet and ruled every five feet
q Compressive strength cylinder tests approved. One test required for every 50 cubic yards or one test for each days pour if less than 50 cubic yards.
q Slump tests approved. Required the same as cylinder tests.
q Air-entrainment test approved. Required the same as cylinder tests.
q Valve
boxes and sewer manholes raised to grade
Final Grading and
Cleanup:
q Backfill between curb and gutter and sidewalk
q Backfill behind sidewalk and grade slope
q All construction debris cleaned up including excess concrete dumps, especially in the city’s right-of-way and dirt tracked into the street.
q All plan changes have been submitted to design engineer for As-Builts.
Final Approval and Acceptance:
q As-built plans, drawings & specifications submitted in plan, booklet and electronic format to City Engineer.
q Letter of acceptance signed by City Engineer.
